Double-incision double-ellipse technique with nipple grafts
In selected patients, Dr Sun uses a double-incision double-ellipse breast-reduction style design with free nipple-areola grafts. The aim is to remove breast tissue and excess skin while shaping the scar and lateral excess in a way that suits the standing chest contour.
A key design principle is to channel the dog-ear tendency toward the axilla, where residual fullness or scar extension may be less conspicuous in a natural arm-down position. In contrast, some standard double-incision designs may push excess more laterally across the chest wall, which can lead to extended lateral chasing of dog-ears and residual standing-visible bunching.
Scar position along the pectoralis major border
Scar position is planned horizontally along or near the lower border of the pectoralis major muscle when anatomy allows. This aims to let the scar sit in a more anatomically expected chest line rather than appearing as an arbitrary straight line across the torso.
If the pectoralis major becomes more developed with training, a scar placed in relation to its border may be visually softened by the natural contour shadow. Scar visibility still varies with skin type, tension, healing biology, hypertrophic scarring tendency and post-operative care.
Central inverted-V scar concept
Instead of a completely straight central scar across the chest, a central inverted-V configuration may be used in selected patients. The purpose is to make the central scar transition appear more anatomical in relation to the pectoralis major border and chest midline.
This is a planning concept rather than a universal rule. The final scar design depends on breast footprint, skin excess, chest width, inframammary fold position, asymmetry and how safely the tissues can be closed.
Small nipple-areola graft planning
For double-incision top surgery, the nipple-areola complex is commonly resized and transferred as a free graft. Dr Sun may plan a small nipple-areola graft, around 2 cm in selected patients, to better match a masculine chest aesthetic.
Nipple grafts require careful handling and post-operative care. Possible limitations include partial graft loss, pigment change, flattening, asymmetry, altered or reduced sensation, delayed healing and need for revision.
Nerve blocks and pain-minimising anaesthesia planning
Pain control is planned with the anaesthesia team. In suitable patients, regional nerve blocks such as parasternal blocks and pectoral nerve blocks may be used to reduce early post-operative pain and opioid requirement.
Nerve blocks do not remove all discomfort and are not suitable for every patient. Pain experience varies, and patients still need clear instructions for medications, dressings, movement, drain care if used and when to seek help.
Day-surgery pathway in suitable patients
FTM top surgery may be performed as a day-surgery procedure in selected medically suitable patients when anaesthesia recovery, pain control, mobility, wound checks and home support are appropriate.
Some patients may require longer observation or admission depending on medical conditions, extent of surgery, bleeding risk, drains, anaesthesia recovery, pain, nausea, travel arrangements or surgeon/anaesthetist advice.
Consultation and preparation
Consultation includes discussion of goals, chest examination, tissue and skin assessment, scar expectations, nipple size and position, medical history, smoking or vaping, medications, mental-health support, documentation requirements, fertility or hormonal considerations where relevant, and recovery logistics.
Patients should discuss whether they are planning future body-building or weight change, because pectoralis development, weight fluctuation and skin quality may influence long-term contour and scar appearance.
Recovery after FTM top surgery
Recovery may involve dressings, compression, limited arm movement, time away from work or school, avoidance of heavy lifting, scar care and follow-up for nipple graft checks. Swelling and bruising are expected early and contour continues to settle over months.
Patients should arrange support for transport and early home recovery, especially if drains are used or if arm movement is restricted. Return to exercise should be staged and guided by clinical review.
Risks and limitations
Risks include bleeding, haematoma, infection, seroma, wound breakdown, delayed healing, visible or hypertrophic scars, dog-ears, contour irregularity, asymmetry, nipple graft loss or pigment change, altered sensation, residual tissue, dissatisfaction, anaesthetic risks and need for revision surgery.
